This page allows for a Public User to search enrolled practitioners. User can enter search criteria, select preferred sort method, and click the Find Now button to find enrolled Medicaid providers.

Practitioner Search
Allows the user to enter enrolled practitioner's NPI/License Number/Provider Name (First Name, or last name, or both) to perform enrolled practitioner search.

| NPI | The 10-digit NPI of the practitioner |
| License Number# | License Number of the practitioner |
| last Name | The Last Name of the practitioner. If only searching by last name, you must enter at least 3 characters. |
| First Name | The FirstName of the practitioner. If only searching by firstname, you must enter at least 3 characters. |
| Sort By | Select how you would like to view the results sorted. |
| Find Now | Select the Find Now button to begin the search. |
| Clear | Select the Clear button to reset the search criteria. |
