This page allows you to view the results of the enrolled Medicaid and Health Choice practitioner search.
Note: Depending on your user role and system access level, you might not see all of the fields listed below.
Allows the user to view the enrolled practitioner search results page by page.
| Field | Description |
|---|---|
| NPI | The 10-digit NPI of the practitioner |
| Provider Name | The name of the practitioner. |
| Health Plans | The health plans (Medicaid and/or Health Choice) in which the practitioner is active as of current date. |
| Address | The practitioner's addresses. |
| Taxonomy | Taxonomy code and description in which the practitioner is active as of the current date. |
| License Number | The license number of the practitioner. |
| OPRLite/OOSLite Status |
If OPRLite, the individual provider is enrolled as an Ordering, Prescribing, Referring (OPR) Lite provider. If OOSLite, the individual provider is enrolled as an Out-of-State Lite provider. All others providers will read as NotApplicable. |
