Tailored Plans and Tailored Care Management (TCM) play crucial roles in providing comprehensive healthcare services to members. With Tailored Plan launch scheduled for July 1, 2024, this course aims to equip Primacy Care Providers (PCPs) and specialty providers with the necessary knowledge and understanding of Tailored Plans and TCM, enabling them to deliver and coordinate enhanced care to their patients.

Learning Objectives

  1. Define Tailored Plans and explore eligibility criteria.
  2. Understand the process and implications of members transitioning between Tailored Plans and Standard Plans.
  3. Explore the relationship between TCM and Tailored Plans.
  4. Define TCM and identify its benefits for members.
  5. Discuss TCM eligibility criteria and the process of opting in or giving consent to participate and how to identify TCM members from the enrollee report.
  6. Examine how TCM benefits PCPs and specialty providers by extending care for patients
