Electronic Funds Transfer Attestation Added to Reverification and Manage Change Request Provider Applications

Effective Dec. 7, 2025, The Electronic Funds Transfer (EFT) Attestation will be added to the reverification provider application and to full Manage Change Requests (MCRs) when active EFT information is present. This attestation requires a signature from the Office Administrator (OA) on the provider’s record.
