Prior Authorization Inquiry Updates

For Prior Authorization (PA) requests submitted on or after June 14, 2026, enhanced decision information will be available in the PA inquiry results within the NCTracks Provider Portal. 

Updates include:

  • Availability of provider issued letters for approvals, voids, adverse decisions and requests for additional information.
  • Display of high-level status reason information for finalized PA records
  • Availability of decision information for PA records transmitted to NCTracks from State Utilization Management (UM) vendors and Managed Care Prepaid Health Plans (PHPs)

Provider issued letters can be accessed through the new PA Letters: View Documents link. It is located under the PA Documents: View Documents link in the top section of the inquiry results. The link will only be displayed if letters are available for the PA record. Availability is based on PA submission date (on or after June 14, 2026) and PA status (involves issuance of a letter).

Selection of the link will open the NCTracks Electronic Document Viewer tool. Providers can view a letter by selecting a FileName in the Document List of available letters. Below is a crosswalk of the PA decision or outcome to letter File Name(s):

  • Approved – DMA1000, DMA1000-A, DMA3504, DMA3504E
  • Change To Previously Authorized Service – DMA1001
  • Denied, Modified, Reduction – MCAID2001-P01, MCAID2001E-P01, MCAID2002-P01, MCAID2002/E-P01
  • Request For Additional Information – DMA3501, DMA3501E
  • Void – DMA3356

For all PAs in a final decision, a high-level reason will be displayed for each line in the PA record. The Reason field is below the Status field and similar to the existing functionality for display of a Reason for Void status. As with the display of provider issued letters, Reason information will only be available for PAs submitted on or after June 14, 2026, and in a final status.

Beginning in July 2026, a list of services requiring prior authorization will be posted to the NCDHHS website Prior Approval and Due Process | NC Medicaid. The list will be available for viewing or downloading in PDF and Excel format.
