NCTracks Contact Us Link Error and Temporary Workaround

NCTracks is aware of an issue causing some users to receive a 404 error when attempting to access or submit requests via the ‘Contact Us’ link on the provider portal and the public NCTracks page. The technical team is actively working on a fix for this issue. As an alternative, providers can send an email directly to nctracksprovider@nctracks.com with one of the following subject options in the subject line:

• Request Additional Training
• Request Customized Training
• Request a Site Visit
• Report Fraud And Abuse
• Technical Website Issue
• Other Request/Question
• 1099 Not Received
• MFA Issue
