Audit Summary Reports for FQHCs/RHCs

Federally Qualified Health Centers (FQHCs)/Rural Health Clinics (RHCs) commonly request Medicaid Audit Summary Reports to facilitate their completion of annual Medicaid cost reporting and Prospective Payment System (PPS) Reconciliations. These audit summary reports are not automatically generated by the fiscal agent. FQHC and RHC providers desiring these reports must use the request process. 

To request a Medicaid Audit Summary Report, fill out the NCTracks FQHC RHC Report Request Form, which can be found under the heading Provider Forms on the Provider Policies, Manuals, Guidelines, and Forms page of the NCTracks Provider Portal. Mail the completed form with a certified check to NCTracks, as explained on the form. The audit summary report will be generated in PDF format within one checkwrite of receiving the request and will be delivered on password-protected CD via FedEx. Signature will be required for the package. The provider can then call the NCTracks Call Center to request the password and the financial team will return the call within one business day to provide the password. This approach is designed to help ensure that the report is only accessible to the provider requesting the report.

There is no limit to the size of the report or the number of reports a provider can request; however, each request must be submitted on a separate form. Service dates must be limited to a 12-month time period. Payment dates can be for a longer period of time.

The FQHC/RHC audit summary reports available from the fiscal agent contain Medicaid data only.  If providers need to request comparable data to facilitate completion of North Carolina Health Choice PPS Reconciliations, please contact the Division of Health Benefits, Provider Audit Section at Medicaid.NCHC.PPS.DataRequest@dhhs.nc.gov.
