Individual Enrollment Types Now Available for Certain Optical and Hearing Aid Taxonomies

The taxonomies listed below have been available to providers as Organization enrollment types but were intended to be for Individual enrollment types, only.

Individual enrollment types have now been added to NCTracks for these taxonomies. Impacted individuals should enroll with them as soon as possible.

The use of the individual taxonomies listed below by organizations will cease in the near future, and the active individual taxonomy will be end dated on the organization’s record. Therefore, organizations or groups who are enrolled with one of the individual taxonomies below, must ensure they have another appropriate active organization taxonomy on their NCTracks enrollment record. Providers who do not, must submit an NCTracks manage change request (MCR) to add an organization taxonomy to their enrollment record. The Provider Permission Matrix (PPM) should be consulted to determine enrollment requirements for specific taxonomies. The PPM is located on the NCTracks Provider Enrollment page.
