Federal Site Visit Requirement Added for Taxonomy 251B00000X (Case Management)
Effective Date: Aug. 24, 2025
According to § 108C-3, "agencies providing nonbehavioral health home- or community-based services pursuant to waivers authorized by the federal Centers for Medicare and Medicaid Services under 42 U.S.C. § 1396n(c)" are considered high risk and require a federal site visit.
The NCTracks Provider Permission Matrix (PPM) has been updated to reflect this requirement.
What’s Changing
Effective Aug. 24, 2025, the Federal Site Visit Required (FEDERAL-SITE-VISIT-RQRD) field in the PPM will be updated from NO to YES for taxonomy code 251B00000X (Case Management).
This change only applies to:
- Organizations and Atypical Organizations classified under “Health Plan Medicaid”
- Organizations and Atypical Organizations under Service Type Community Alternatives Program for Children (CAP/C), CAP CONSUMER DIRECTION SERVICES AND Community Alternatives Program for Disabled Adults (CAP/DA)
