New Text on Accreditation and Federal Regulations Pages of NCTracks Provider Enrollment Applications for Physical Therapists
Physical Therapists enrolling with North Carolina Department of Health and Human Services (NCDHHS) programs under taxonomy 225100000X have a moderate categorical risk which requires a site visit. Unless the individual Physical Therapist is operating as a sole proprietor and billing under their individual National Provider Identifier (NPI), the site visit is performed on their affiliated organization location when that location has not had a site visit within the previous five years. This special circumstance requires a manual review to determine the appropriateness of a site visit. For this reason, additional guidance is being offered on the NCTracks application.
Beginning Aug. 10, 2025, new static text will be added to the Accreditation and Federal Requirements page for all application types (Initial, Re-enrollment, Managed Change Request and Reverification) enrolling with Individual taxonomy 225100000X - Physical Therapist.
On the accreditation page, in the License section which identifies the license type needed for the taxonomy, the following note will be added as an additional bullet point when requirements are displayed for the 225100000X - Physical Therapy taxonomy:
“This taxonomy requires a site visit. When you reach the Federal Requirements Page of the application, please check NO to question "Have you completed the Federal site visit for this site to NC Medicaid, another state or Medicare?" when affiliating with an organization. This will cause a manual review to prompt a site visit only when confirmed necessary."
On the Federal Requirements page, in the Federal Site Visit static text box, the following text will be added:
“Taxonomy 225100000X-Physical Therapy requires a site visit. If enrolling with this taxonomy, please check NO to question "Have you completed the Federal site visit for this site to NC Medicaid, another state or Medicare?" when affiliating with an organization. This will cause a manual review to prompt a site visit only when confirmed necessary."
