This training is part of a series designed for Primary Care Providers interested in becoming Advanced Medical Homes in NC Medicaid Managed Care.

This webinar will provide an in-depth look at how AMH practices will manage patient assignment, empanelment and assessment processes. We will review the beneficiary primary care provider (PCP) and Prepaid Health Plan (PHP) selection processes and consider how these relate to AMH practice requirements. Then, we will detail AMH Tier 3 risk stratification and the Comprehensive Assessment requirements, and how these processes are linked to the identification of high need beneficiaries in need of care management and inform the development of AMH Care Plans. Finally, we will conclude with a set of practice “use cases” that will provide examples of how Tier 3 practices may choose to partner and delegate AMH responsibilities to Clinically Integrated Networks (CINs) or other partners.

This webinar will provide a high-level overview of the transition of current day Pregnancy Medical Home (PMH), Obstetric Care Management (OBCM) and Care Coordination for Children (CC4C) programs to managed care. It will discuss the role of maternity and pediatric providers, Local Health Departments (LHDs), and PHPs in continuing to care for high-risk pregnant women and at-risk children ages 0-5 under managed care.

The webinar will lay out how management for these populations fits into the broader managed care transition, the key elements of each program, and the infrastructure for program oversight and accountability.
